Driver for Compaq FC Card?

We want to use some old hardware (fc-card,raidarray) for a testserver (DL360G3).

I need a driver for the following Compaq FC Card:

66MHz 64bit PCI controller

The biggest chip is Agilent Tachyon TS HPFC-5166B/2.2 L2E1090 J 0221

On the other side:

I do not know the vendor of the card.
The Raid Array is a Compaq StorageWorks 4100.

Can you please tell me the vendor and model to this FC Card?
